We have a dedicated team of healthcare professionals who are assigned to each client to ensure that his or her needs are met. Our team will also provide an admission conference, education and information to both the client and in-home caregiver to better ensure their participation in the Plan of Care.
Clients will be assessed by either their Physician or their Registered Nurse (on initial admission assessment) for a need for Physical Therapy, Speech Therapy, and/or Occupational Therapy.

Medicare Home Health Benefits Eligibility
The following is an excerpt from the booklet published by Medicare called Medicare and Home Health Care that explains the rules for Medicare Home Health Care eligibility and covered services.
If you have Medicare, you can use your home health benefits if you meet all the following conditions:
A person may leave home for medical treatment or short, infrequent absences for non-medical reasons, such as attending religious services. You can still get Home Health Care if you attend adult day care, but you would get the home care services in your home.
